Family caregivers can help shape the outcomes for their loved ones – an ICU nurse explains their vital role

Marie landed in the intensive care unit where I am a bedside nurse because she was too agitated and needed more oxygen.

Key Points:
  • Marie landed in the intensive care unit where I am a bedside nurse because she was too agitated and needed more oxygen.
  • As a seasoned bedside critical care nurse, I see firsthand the benefits that family caregivers bring to patient care in the hospital.
  • I also witness the emotional stress that caregivers experience when their loved one comes to the ICU.

Caregivers often battle anxiety and depression

  • From making medical decisions to advocating for their loved one, family caregivers have many important roles when their loved one is in the hospital.
  • However, caregivers carry heavy emotional burdens while supporting loved ones at the hospital.
  • Up to 70% of first-degree relatives of ICU survivors suffer from anxiety symptoms, more than a third suffer from depression, and many can experience symptoms of post-traumatic stress disorder, or PTSD.

Communication is critical

  • As you provide information about the patient at home, the medical team should be giving you updates about the patient’s condition in the hospital.
  • Knowing what to ask is essential to effective communication at the hospital.
  • Once familiar with your new environment, you may feel more at ease to truly be present for your loved one.
  • Every day, the interdisciplinary medical team sees each patient to discuss updates and treatment plans, and answers questions for the patient and their family.
  • Research has also shown that rounds relieve anxiety and stress among family caregivers due to the consistent daily communication and emotional support that they provide.

Nurses can be helpful

  • After clinical rounds, the interdisciplinary team of doctors and nurses establishes a daily plan of care for your loved one, which will be carried out by your bedside nurse.
  • The nurse will give the ordered medications, perform necessary clinical tasks and assess the patient for their response to the treatment.
  • Nurses are the most accessible resource you have when your loved one is in the hospital.

The importance of routines and familiarity

  • Preventing ICU delirium through reorientation and familiar faces can help prevent this serious complication and can even reduce their hospital stay.
  • Most families that come to the hospital describe the support they received from the medical team in a positive way.

West Africa’s falling fish stocks: illegal Chinese trawlers, climate change and artisanal fishing fleets to blame

Retrieved on:
Tuesday, April 9, 2024

Average fish catches by traditional fishing communities along the west African coast have declined significantly over the past three decades.

Key Points:
  • Average fish catches by traditional fishing communities along the west African coast have declined significantly over the past three decades.
  • They have done this for generations, but since the 1990s, a decline in the catch has put their livelihoods at risk.
  • In Ghana, total landings of small pelagic fish fell by 59% between 1993 and 2019, despite increased fishing efforts.
  • The continuing decline in fish catches has serious implications for some of the poorest families in the region.

Climate

  • Among the multiple threats from climate change, ocean warming is probably the least appreciated.
  • This helps contain warming on land in the short run, but in the long run it brings a cascade of larger climate threats.
  • The models are widely used to forecast range shifts of organisms due to climate change and predict the eventual ranges of invasive species, among others.
  • It projected that climate change alone would reduce maximum catch potential in the Guinea Current System by 30% or more by 2050, even if the fisheries were well managed.

Chinese trawlers

  • However, Chinese trawlers get around this barrier by using local companies as legal “fronts”.
  • Chinese companies, thinly disguised as Ghanaian companies, currently own over 90% of Ghana’s licensed bottom trawlers.
  • Chinese trawlers are occasionally fined for illegal practices in Ghana, but some fail to pay the fines and still do not lose their licence.

Too many canoes

  • In west Africa there are now seven times as many canoes engaged in ocean fishing as there were in 1950.
  • Today’s canoes have larger nets and bigger crews, and many have powerful outboard engines.
  • Despite the recent fish catch decline, canoe numbers have continued to increase; in Ghana there were 8,000 canoes in 1990, but by 2017 there were 13,650.

New livelihoods

  • This won’t be easy since roughly 40% of coastal fishermen in Ghana and Nigeria have no formal education.
  • One policy measure to keep them in school would be to provide monthly cash transfers conditioned on school enrolment and attendance.
  • For fishing communities threatened by falling fish stocks, this might be a path to future livelihood protection.

Key Points:
  • Knowing that her breast milk was the best nutrition for Charli, Moultrie tried pumping to get her milk supply started.
  • After two weeks of consistently pumping with scant results, Moultrie was grateful to learn that Charli could receive safe, pasteurized human milk through the hospital's donor milk bank.
  • The milk bank also has a legacy program for women who have lost a baby but want to donate their milk to help other infants.
  • “But there are times around the holidays and during the summer when we only have enough milk for hospitalized babies.”
This means the milk bank is not always able to provide milk to babies who have been discharged from the hospital with a doctor’s prescription for pasteurized donor human milk.
